I tested on Singleplayer and did around 50-75 enchants of fishing rods and it never showed a blank enchant in the first slot, but it showed a blank enchant in the first slot at least 4 times within 20-25 enchants on my Spigot server.

How to recreate:

Keep enchanting fishing rods until you see the first slot is blank in the enchantment table.
