When using no plugins, both on latest Spigot `3032-Spigot-1ad8354-b530534 (MC: 1.16.5)` as well as CB `3032-Bukkit-b530534 (MC: 1.16.5)`, when `Accessibility Settings -> Narrator: Narrates Chat` is set, it doesn't narrate the chat.
The issue is not present in Vanilla 1.16.5 (both SP as well as using server jar).
Also tested 1.16.4 and 1.15.2 CB jars, with the same faulty behavior.

If the setting is set to `Narrator: Narrates All` instead, it successfully narrates chat.

Using chat plugins also seems to fix the issue - tested using fresh install of EssentialsX Chat v2.18.2.0 on 1.16.5 CB, the behavior is correct.
